Stillwater Medical Center has been here for decades, caring for generations of families. As our community grows, more help will be needed to provide the best care. A bequest can create a living legacy and provide high-quality care for friends and family.

Foundation staff can provide information about areas of care you may wish to remember in your will.

By leaving a gift to the hospital, you are, in essence, helping all of your friends.

Other Important Documents

The Oklahoma State Department of Heath (OSDH) provides an  Advance Directive For Healthcare. This form lets you explain your desires for end-of-life care. Best practice is to give a copy to your physician, your attorney, and your health care proxy This is because it needs to be easy to access. The Advance Directive for Healthcare requires two witnesses.

Additionally, the OSDH has a Durable Power of Attorney (With Healthcare Powers Only) form. This type of advance directive can become effective immediately or when your attending physician determines you are no longer capable. This is not the same as a General or Limited Power of Attorney. The Durable Power of Attorney (With Healthcare Powers Only) requires two witnesses and a notary.
